Registration Form for Shooting the Stars Night Photography Workshop, June 29-30, 2019

 

To reserve your space in this workshop, please fill out the form below, then carefully read the terms and conditions. You must check the box indicating acceptance of the terms and conditions to proceed with registration. Your agreement to the terms of the liability waiver is also required. Please read the waiver carefully, then check the box signifying your acceptance of its terms.

 

After checking both boxes, please click the button at the bottom of the page labeled "proceed to payment." If you are registering before May 30th, 2019, you must pay a deposit of $150 per person. Your space in the workshop is not guaranteed until Glenn Randall Photography receives your deposit. If you are registering on or after May 30th, 2019, you must pay the full workshop tuition of $475 per person.

 

If you register before May 30th, 2019, the balance of $325 per person will be due no later than midnight on May 30th, 2019, 30 days in advance of the workshop. Glenn Randall Photography reserves the right to cancel your enrollment in the workshop and fill your spot from the waiting list if full payment is not made by that date. No refund of the deposit will be given in this circumstance. Glenn Randall Photography will send a reminder two weeks in advance of the deadline for final payment.

Terms and Conditions for the June 29-30, 2019, Shooting the Stars workshop

 

Cancellation policy: If you cancel before midnight on May 15th, 2019 (45 days before the workshop), you will receive a full refund of all money paid. If you cancel between midnight, May 15th, 2019, and midnight, May 30th, 2019 (less than 45 but more than 30 days prior to the workshop), you will receive a refund of all money paid minus a $150 cancellation fee. If you cancel after midnight, May 30th, 2019, (less than 30 days before the workshop), you will receive no refund unless Glenn Randall Photography can fill your spot, in which case you will receive a refund of money paid minus a cancellation fee of $150. If you do need to cancel, please let Glenn Randall Photography know as soon as possible so that people on the waiting list can have the opportunity to take the workshop.

 

Life happens, and the best-laid plans can go awry. Glenn Randall Photography strongly recommends purchasing trip cancellation insurance. The cost is nominal compared to the total cost of the trip. Please check out the following two websites, where you can research and purchase travel insurance.

In the highly unlikely event that Glenn Randall Photography cancels this workshop, all money paid will be refunded. Under no circumstances, however, will Glenn Randall Photography be responsible for or be held liable for any costs incurred by you involving your planned participation in this workshop, including, but not limited to: all travel-related costs, camera & other photography-related equipment purchases or rentals, purchases of clothing, luggage, etc. All workshop schedules are subject to change at the sole discretion of Glenn Randall Photography due to inclement weather, health concerns, safety considerations, or other factors.

 

VISITOR’S ACKNOWLEDGEMENT OF RISKS

 

In consideration of the services of Glenn Randall Writing & Photography, their officers, agents, employees, and stockholders, and all other persons or entities associated with those businesses (hereinafter collectively referred to as “Glenn Randall Photography”) I agree as follows: Although Glenn Randall Photography has taken reasonable steps to provide me with appropriate equipment and skilled guides so I can enjoy an activity for which I may not be skilled, Glenn Randall Photography has informed me this activity is not without risk. Certain risks are inherent in each activity and cannot be eliminated without destroying the unique character of the activity. These inherent risks are some of the same elements that contribute to the unique character of this activity and can be the cause of loss or damage to my equipment, or accidental injury, illness, or in extreme cases, permanent trauma or death. Glenn Randall Photography does not want to frighten me or reduce my enthusiasm for this activity, but believes it is important for me to know in advance what to expect and to be informed of the inherent risks. The following describes some, but not all, of those risks. Rocky Mountain National Park, Arches National Park, Canyonlands National Park, Dead Horse Point State Park, Uncompahgre National Forest, and other areas where workshops may be conducted represent a potentially dangerous natural environment. Potentially hazardous areas include roadways, trails, waterways and un-manicured natural areas. Potential hazards in these areas include cliffs, swift rivers and streams, lakes, severe weather, and local plant life and wild animals native to the area, exposure to which may result in disease, serious injury or death. This activity also requires a low to medium level of physical exertion which may also result in injury or death for those who are not in adequate physical condition to withstand such exertion. This activity takes place in a remote area where emergency services may not be readily available, and the environment may hamper attempts to assist and/or rescue those who have been injured, and such delays may result in further injury or death. I am aware that participating in a Glenn Randall Photography workshop entails risks of injury or death to any participant. I understand the description of these inherent risks is not complete and that other unknown or unanticipated inherent risks may result in injury or death. I agree to assume and accept full responsibility for the inherent risks identified herein and those inherent risks not specifically identified. My participation in this activity is purely voluntary, no one is forcing me to participate, and I elect to participate in spite of and with full knowledge of the inherent risks. I acknowledge that engaging in this activity may require a degree of skill and knowledge different than other activities and that I have responsibilities as a participant. I acknowledge that the staff of Glenn Randall Photography has been available to more fully explain to me the nature and physical demands of this activity and the inherent risks, hazards, and dangers associated with this activity. I certify that I am fully capable of participating in this activity. Therefore, I assume and accept full responsibility for myself, including all minor children in my care, custody, and control, for bodily injury, death or loss of personal property and expenses as a result of those inherent risks and dangers identified herein and those inherent risks and dangers not specifically identified, and as a result of my negligence in participating in this activity. I have carefully read, clearly understood and accepted the terms and conditions stated herein and acknowledge that this agreement shall be effective and binding upon myself, my heirs, assigns, personal representative and estate and for all members of my family, including minor children.
